Resources for Families Enrolled in Our Childcare Center

Make sure you check out our parent website at shutterfly.  Your child’s classroom teacher can give you the password to access the site.  Here you can check out the center’s events calendar as well as pictures of our activities at Renaissance and news from the classrooms.  It’s a great way to keep tabs on what your child is working on while you are at work!

Do you know someone looking for childcare?  You can earn free daycare for your children by referring them to Renaissance.  If you refer someone to Renaissance and their child enrolls for at least three months you earn a free week of daycare for your child!  It’s that easy and there’s no limit on how many families you can refer!  Help Renaissance and your pocket book grow by referring someone today!

Schedule Requests for the Director

Click here to request a permanent change in your child’s weekly daycare schedule (example: switching from attending Mondays every week to Fridays instead)

Click here to request a vacation day from daycare for your child. Make sure your request is submitted at least 10 days before the date you are requesting.

Click here to make a one-time extra hours request for your child's daycare schedule (example: This Thursday your child does not usually attend but you had an emergency come up and you need him/her to attend this one time).

Childcare Enrollment Forms
Here you can download and print all of the forms we need for your child. Make sure to keep us updated whenever you have an address or phone number change. Below you will find all of the forms available to parents with a description of what it is used for:

• Childcare Enrollment Form: Basic enrollment information completed at time of initial enrollment
• Emergency Contact Card: This card contains information on who to contact in case of an emergency as well as specific instructions for handling any special medical conditions/allergies your child may have as well as the names of individuals other than the parents who can pick up your child from daycare.
• Health Report: This form is required by the State of Wisconsin within 3 months of your child’s enrollment at Renaissance. Your doctor signs the form and indicates that your child is healthy enough to be in a group care setting like a childcare center. Take this form to your doctor every time your child has a checkup and have a new one completed and returned to the office.
• Immunization Record: This form allows you to indicate your child’s immunization record. A Printout of that record from your pediatrician is also acceptable.  Keep this form updated with the office.  Like all childcares, we are required to report immunization statistics to the State annually.
• Photo Permission / Field Trip Permission Form: You can identify when it is acceptable for Renaissance to use a picture of your child by filling out this form.  The bottom portion of the form allows us to take your child on field trips.  While major field trips are announced well in advance, we also like to take walks around the neighborhood which also count as field trips.
• Food Service Agreement: This form is used to ensure that you are aware of the center policies pertaining to the free meals we provide to your child.
• Income Statement : This form is required by the USDA and allows Renaissance to provide meals to the children.  Your information is kept confidential and is viewable only by the administrator.  We understand the importance of privacy and respect your right keeping your information private and secure.
• Intake Form : This form helps us understand where your child is developmentally. This form is required to be filled out every for children under the age of 2 years. Parents must fill out the form initially.  Updates every 3 months are completed by the teaching staff.
• Infant Formula Letter : This form is required for infants only (under 12 months) and tells us who will be providing milk to your child.  The selection made on this form can be changed at any time (breast milk, formula from home, formula from the center)
• Jar Food/Cereal Letter : This form is required for infants only (under 12 months) and tells us who will be supplying jar foods and infant cereal to your baby.  The selection made on this form can be changed at any time.
• Credit Card Payment Authorization Form : Use this form to set up automated credit card payment of your childcare bill at no extra cost!
• Automatic Checking Payment Authorization Form : Use this form to set up automated checking withdrawal payments of your childcare bill.
